The reported case

On 7 October 2026, HSE reported that Fitzpatrick Fabrications Ltd had been fined £48,000 following a serious hand injury during a manual handling task. The incident occurred in October 2024. HSE said a near-identical incident had happened in the preceding days and that the task had not been risk assessed or supported by a safe system of work.

The company pleaded guilty to a breach of the Manual Handling Operations Regulations 1992. Sentencing took place on 6 October 2026. These are the findings reported by HSE; the linked release contains the full account.

Our reading: close the loop

The wider lesson is about the response between an early warning and the next attempt at the job. A near-miss form is a starting point. It cannot, by itself, change how a heavy or awkward component is supported, moved or repositioned.

For a site manager, a useful review starts with one recently reported problem. Ask what action was agreed, who had authority to change the work, and how the team knew what to do next. If those answers live in separate conversations, bring them together.

A small review with a clear outcome

Choose a task with a recent warning or recurring difficulty. Talk to the people carrying it out and the competent person responsible for assessing it. Understand the actual sequence, including setup, repositioning and clearing away.

Record the agreed changes, any restrictions while they are outstanding, the responsible person and the evidence required before closure. If suitable controls cannot be confirmed, escalate the task through the site’s safety arrangements. A fresh signature on the old method is not evidence that the problem has been resolved.
